The numbers, worked
Bank payments charge a percent with a cap, so on large invoices the fee stops growing. Stripe lists ACH Direct Debit at 0.8% capped at $5.00, PayPal lists Pay by Bank on an invoice at 1% capped at $10.00, and Square lists a bank transfer on an invoice at 1% with a $1 minimum and a $10 cap.
Stripe ACH on $5,000.00: 0.8% would be $40.00, so the $5.00 cap applies and you invoice $5,005.00. PayPal Pay by Bank: 1% would be $50.00, capped at $10.00, so you invoice $5,010.00.

Questions
When does the cap start to matter?
Stripe's 0.8% reaches $5.00 at a $625 payment; PayPal's and Square's 1% reach $10.00 at $1,000. Above those amounts the fee stays the same.
Why does the calculator add exactly the cap?
Once the fee is capped it no longer depends on the invoice, so the invoice is the amount you want plus the cap.
